MVMS Afterschool Sign-Up Form

All students staying after school for clubs must complete the Afterschool Sign Up Form. All clubs will end at 4:00 unless otherwise noted by the advisor. There is no late bus. We kindly request that parents pick up their child at the end of their club session.  Student Athletes staying after for practice or a game do not need to sign the form.
